Why the family history matters most
The most important part of a sports physical is not the stethoscope — it is the questions about your family. A history of sudden cardiac death, fainting during exercise, or unexplained early heart disease in a relative changes what we look for. Bring what you know about grandparents, parents, aunts and uncles.
Bring your school form
Most Oklahoma schools and leagues use their own form. Bring it with you and we complete and sign it during the visit so nothing is left for later. If you forget it, we can provide a standard clearance form.
Best time to come
August and early September are the busiest weeks of the year for these exams. If your child plays a fall sport, coming in July or early August means far less waiting.

$39 for a child paying cash. That is the full price, including completing the school form.
Do I need to bring the school form?
Yes, bring it if your school gave you one — we fill it out and sign it during the visit. If you do not have one, we can provide a standard clearance form.
How long does it take?
About twenty to thirty minutes for a straightforward exam.
Can this count as my child’s yearly checkup?
A sports physical is focused on safe participation, not overall health. A full well-child visit covers growth, development, vaccines and screening, and is a separate, more thorough exam. We are happy to do both in one visit.
